Introduction : La dacryocystorhinostomie (DCRS) est basée sur la création d’une communication permanente entre le sac lacrymal et la fosse nasale correspondante par le moyen d’une fenêtre osseuse ouverte à travers la gouttière lacrymale située sur la paroi externe de la fosse nasale. Le but du travail était d’illustrer l’expérience de notre service en matière de pratique de la DCRS
Matériels et méthodes : Etude rétrospective, colligeant 15 dacryocystorhinostomies, faites par voie endonasale, entre janvier 2014 et janvier 2015, réalisées par une double équipe ORL-ophtalmologistes.
Résultats : L’âge moyen a été de 37 ans, avec prédominance féminine, trois patients avaient des antécédents de dacryocystorhinostomie externe. Tous nos patients avaient un larmoiement chronique. L’examen ORL a objectivé une déviation septale chez sept patients et une hypertrophie du cornet inférieur chez deux patients. L’examen ophtalmologique a objectivé des voies lacrymales obstruées et un bon contact osseux chez tous les patients. La tomodensitométrie de la face a été réalisée d’une façon systématique. La DCRS a été associée à une septoplastie chez sept patients et à une turbinectomie moyenne chez un patient. La sonde bi-canaliculaire a été posée systématiquement chez tous nos patients. L’évolution a été marquée par un succès anatomique et fonctionnel chez 93% des patients.
Conclusion : La DCRS doit être l’intervention chirurgicale de première intention pour traiter les obstructions basses des voies lacrymales, et non une technique de reprise. Elle permet de traiter au cours du même temps opératoire des pathologies nasales associées et évite l’incision cutanée externe inesthétique et ses complications. Les clés du succès sont la transillumination et le maintien de la sonde bi-canaliculaire durant au moins 3 mois dans le néo-ostium.
Introduction: Dacryocystorhinostomy is based on creating a permanent communication between the lacrimal sac and the corresponding nasal cavity by means of a bone window opened through the lacrimal fossa located on the outer wall of the nasal cavity. The Aim of study is to illustrate the experience of our service for the practice of endonasal dacryocystorhinostomy and compare our results with those reported in the literature.
Materials and methods: A retrospective study, which concerned 15 dacryocystorhinostomies made by endonasal technique between January 2014 and January 2015 carried out by an ENT-ophthalmologists double team establishing a bicanalicular probe for 6 months.
Results: The mean age was 37 years, female predominant, three patients had a history of external dacryocystorhinostomy. All patients had chronic watery. The ENT examination objectified a septal deviation in seven patients and hypertrophy of the inferior turbinate in two patients. Ophthalmologic examination objectified lacrimal obstructed and good bone contact all patients. Computed tomography of the face was conducted in a systematic way. The RSD has been associated with a septoplasty in seven patients and average turbinate in a patient. The bicanalicular probe was raised systematically in all our patients. The evolution was marked by an anatomical and functional success in 93% of patients.
Conclusion: Endonasal dacryocystorhinostomy with endoscopic guidance should be the first line treatment for low lacrimal obstructions, not a recovery technique. It can handle during the same operation associated nasal pathologies and prevents unaesthetic external incision and its complications. The first key to success is the transillumination, the second key to success is maintaining bicanalicular probe for at least 3 months in the neo-ostium.
